What to do if you change your name

Learn what you need to do if you change your first, middle or last name(s).


We need to know if you change your first, middle or last name(s). This is a legal requirement so that we can accurately process your pension in a timely way.

  • Complete and print the Declaration of change of name (first or middle) form
  • Submit the form to BC's Municipal Pension Plan or update your information in My Account

You can also send us a legal change-of-name document.

If you change your last name:

  • Sign in to My Account to update your personal profile information. You will need to upload an electronic copy of either a marriage certificate or legal change-of-name document as proof of your name change.

You can also tell us of your new last name by sending us a copy of your marriage certificate or legal change-of-name document.
