Learn how to make a claim for eligible expenses covered by your extended health care or dental plan.
You can submit claims for your eligible health care and dental expenses:
- Through your health or dental service provider (such as when your pharmacist or dentist submits on your behalf)
- Online through Member Profile
- By mail to Pacific Blue Cross
